For New Patients

A Patient's first visit to our office establishes a vital foundation for our Patient / Doctor relationship. During the first visit, we will need to obtain important information, such as your medical history and current dental status, but more important, we want to understand your dental concerns and esthetic desires.  At our office, New Patient cleanings are usually scheduled an hour and thirty minutes.  This extra time helps our Staff accurately create your Patient Chart, and allows you to get to know your Doctor, Hygienist, and our helpful Staff.

We strive to maintain consistent care by scheduling patients with the same Doctor and Hygienist whenever possible. Our goal is to be on schedule, barring any dental emergency, because we understand that the patient's time is as valuable as ours. We are bound to continually improve our dental knowledge by attending continuing education courses and learning more about new dental procedures. It is our job to present the patient with the options of modern dentistry. We understand that patients are limited by their financial situations and will always offer alternative treatments when possible, but we will not lower our dental standards due to limitations placed by insurance carriers.

To help make your visit go smoothly, please download the forms below, fill them out in advance and bring them to your appointment.  If you are unable to download, the forms will be available at our front desk at the time of your appointment, or we will be happy to mail them to you.   Forms will need to be completed and signed for each family member.

Our office is conveniently located in the West End / Innsbrook area of Henrico County, just outside of Richmond.  We are only 15 minutes from downtown and 5 minutes from Short Pump Town Center.  Parking is free, and our facility is wheelchair accessible.  Please call for additional directions.
